Supported Living Services

Our services are designed to support care leavers between the ages of 16 – 25 years to build positive support networks, focus on their strengths and achieve their full potential.

We aim is to provide a truly person centered support service tailored around each individual’s needs by working through a structured and personalised support plan which focuses on encouraging the young person’s to achieve their goals and take control of their lives.

We support the young person’s in the following areas;

  • Education and training
  • Work and careers
  • Basic life skills
  • Accommodation and living
  • Health and well being
  • Money, budgeting and finances
  • Family and community
  • People and relationships

We offer young people Move on Services, Floating Support and Community Outreach services to help them with accommodation as well as support services to address everyday issues they might face.

We work to improve outcomes for young people and their families to live happy, healthy and fulfilled lives. We believe in engaging and involving the young persons in every aspect of their support to ensure greater success.

We are there to prepare the young person’s prepare for independent living by offering information on housing options. This will include accurate and relevant information about what is likely to happen when they make a transition to independent living. This will ensure that they are aware and prepared for what happens next and support better outcomes.

Each young person is assigned their own personal adviser and key worker to help guide through the process. They will support the young person on a day to day basis and offer continued support once they have moved onto independent living.
